Required documentation:

  • Copy of a Voided Business Check or Voided Personal Check for Sole Proprietor or Sole Member LLC. If you do not have a voided check, a bank letter can be used. The bank letter needs to be printed on the bank’s letterhead and signed by a representative of the bank, confirming that the business has an account at the bank and includes both the routing and checking account numbers. If you do not have either of these available, please contact us.
  • Two proofs of business which can include any documents issued by the local, state, or federal government with the business name and address. Utility bills and lease agreements are acceptable as well.
  • Copy of your Driver's License, Passport or other government issued photo ID.


Additional Information:

  • IRS Form SS-4, Application for Employer Identification Number (EIN): Is used by businesses, non-profits, trusts, and estates to apply for an Employer Identification Number (EIN). An EIN is a unique nine-digit number issued by the IRS for tax identification purposes, similar to a Social Security Number but for businesses.
  • FFL: Signed file copy of your current FFL if you sell firearms.
  • Photos: If you have a storefront, please provide three photos of your location. One of the exterior with your signage, one of the interior of the store, and a third of any inventory you have.
  • Contracts: If you require your customers to sign a contract for services, please provide a copy of the contract.
